Four forms is a new record, congrats. My school's system was a black hole too, just like @haydenc10 said. I once got a follow-up email asking for more details on my "bias incident" which was just a vending machine eating my dollar. They had a whole committee review the snack theft.

Whoa hold up, I actually kinda see it differently. My school's bias reporting system was surprisingly useful, not a black hole at all. I reported a professor who kept misgendering a classmate and got a call back from the office within 48 hours, and they actually followed up with the department about it. I know a lot of these forms get buried or turned into jokes, but I've seen firsthand where they actually do something when the report has real substance behind it.
