One bad week at my last job taught me more than 2 years of meetings

Honestly, my old boss at that logistics place in Tulsa gave me a performance review that was all fine, then the next week I made one small mistake in a shipment invoice. It was like $300 off, and suddenly I was in three different meetings about it. One guy, Dave from accounting, kept saying 'we need to talk about patterns' when I had zero patterns before. I realized that week that the review meant nothing, they were just looking for a reason to push me out. Has anyone else had a place where one tiny error turned into a whole process against you? What did you do?
