The week I had to redo an entire floor because of a bad pad
Last month in Phoenix, I put down 1200 square feet of a nice berber for a client. The pad felt a bit thin when I was rolling it out, but the box said it was the right spec. A week later, they called me back because the carpet felt loose and you could hear crunching with every step. I had to pull the whole thing up, and sure enough, the pad had basically flattened to nothing. Lost two full days and ate the cost of the new pad. Ever have a material fail that fast on you? What's your go-to brand for padding now?

Oh man, I gotta respectfully disagree though. I've had bonded urethane fail too when it's the cheap stuff from the wrong supplier. For me it's all about density and getting a rebond pad with at least 8 pounds - that's what's held up way better in my jobs.
