Had a close call with a frame rack last Thursday

I was pulling a 2020 F-150 on the rack at our shop in Tacoma. The customer said it was a light hit, but the numbers were off by about 3/4 of an inch. I went to make the final pull and heard a loud pop from the anchor point. The chain had slipped because I didn't double-check the hook was seated fully on the pinch weld clamp. The truck shifted but luckily didn't come off. I had to stop everything, reset all the anchors, and add a safety strap. It added two hours to the job. Anyone else had a rack anchor fail on them?

Glad it was just a scare and nothing worse. I started using a bright zip tie as a visual check on every hook now, just to see at a glance if it's seated all the way. That pop sound is enough to make your heart stop. It's a solid reminder to slow down, even on what looks like a simple pull.

Honestly how bad was it really? A chain slipping and a loud noise sounds scary but the truck didn't actually fall. Isn't that just a normal part of the job that gets fixed with a reset?
